Lifestyle Interventions operates an SMS messaging program to communicate with individuals who have inquired about or are receiving our services. By providing your mobile phone number and opting in through our contact form or another interaction with us, you may receive the following types of messages:

• Transactional messages: appointment reminders, scheduling notifications, follow-up communications, and care coordination related to Family Support, Crisis Interventions, our Wellness Program, Ongoing Monitoring, or Advisory services.
• Promotional messages: service announcements and other marketing communications (only if you have separately and explicitly opted in).

These two message types require separate consent and are managed independently. Marketing consent is never combined with transactional consent.

4. SMS Opt-Out Instructions
